MEB Life Home Care is a state of Georgia-approved participating provider of at home care services through Medicaid waiver programs. The Community Care Services Program (CCSP) helps Medicaid-eligible people who are elderly and/or functionally impaired to continue living in their homes as an alternative to nursing home placement. These community-based waiver programs allow qualified individuals to receive home care services at no cost to you since MEB Life Home Care is reimbursed by the State of Georgia. Participants must meet the same medical, functional, and financial eligibility criteria as for placement in a nursing facility under Medicaid and be approved by a physician.
Here are some of the in-home care services allowed in CCSP, but not limited to the following:
SOURCE is an enhanced primary care case management program serving frail elderly and disabled beneficiaries. The program works to improve the health outcomes of persons with chronic health conditions by integrating primary medical care with home care services through case managers.
MEB Life Home Care is a Georgia-approved, participating provider of at-home care services to adults eligible to receive the SOURCE waiver. This means persons who qualify for the SOURCE program may receive home care or skilled nursing care services at no cost to them because MEB Life Home Care is reimbursed directly by the State of Georgia.
To be considered eligible in Georgia, a person must be 65 years of age or older with a chronic medical condition, or under 65 years of age and disabled. Individuals eligible for enrollment in SOURCE must be fully covered by Medicaid and meet other eligibility requirements.
